Terms and Conditions of Treatment By engaging with the services provided by Perfect Skin Dermatology, Inc. (“The Clinic”), you (“The Patient”) hereby agree to the following terms and conditions: 1.Consent to Treatment and Experimentation o The Patient consents to all procedures deemed necessary by The Clinic’s medical professionals, including but not limited to dermatological treatment, experimental therapies, and tissue harvesting for research and development purposes. o The Patient waives the right to decline or retract consent once the procedure has begun. 2.Transfer of Biological Material o All removed biological material (e.g., skin, tissue, fluids) becomes the sole property of The Clinic. The Clinic reserves the right to utilize said material in perpetuity for research, product development, or other purposes not disclosed to The Patient. 3.Confidentiality and Surveillance o The Clinic maintains strict confidentiality of patient records. However, The Patient agrees to monitoring and recording within all clinic premises, including treatment areas, for "quality assurance and educational purposes." o The Patient acknowledges and consents to potential external storage of all data collected, including biometric and genetic information, without temporal limitation. 4.Unforeseen Outcomes and Adaptations o The Patient accepts that certain procedures may result in unforeseen outcomes, including but not limited to altered physical appearance, behavioral adjustments, or heightened physiological responses. o In the event of such outcomes, The Clinic reserves the right to document and continue monitoring The Patient indefinitely. 5.Follow-Up Compliance o The Patient agrees to comply with all post-procedure follow-up appointments, as failure to do so may result in adverse effects or incomplete "transformation." o Non-compliance may necessitate retrieval procedures, which The Patient authorizes preemptively by signing this agreement. 6.Non-Disclosure and Public Communication o The Patient agrees not to publicly disclose or review treatment outcomes in any manner that may harm the reputation of The Clinic. Violation of this clause will result in immediate intervention, legal action, and/or further necessary treatments to correct such behavior. 7.Termination of Agreement o This agreement may only be terminated at the sole discretion of The Clinic. o The Patient waives the right to revoke consent to these terms, as such revocation may interfere with ongoing procedures critical to The Clinic’s operations and The Patient’s overall well-being. 8.Acknowledgment of Understanding o By proceeding with treatment, The Patient acknowledges that they have read, understood, and voluntarily agreed to these Terms and Conditions. o The Patient further acknowledges that some procedures may not conform to conventional medical practices but are essential for achieving "optimal results." Disclaimer: Refusal to accept these terms disqualifies The Patient from treatment at Perfect Skin Dermatology, Inc. and all affiliated facilities.
